Join us on our journey to develop Active Lifestyle Hubs: venues and exercise professionals who provide support, advice and guidance to people living with long-term health conditions signposted via the Move Together pathway. The hubs and the exercise professionals who operate them will help residents to build movement and physical activity into their lives to improve their physical and mental well-being.

Project Overview

An Active Lifestyle Hub will be a venue or group of exercise professionals providing a range of activity for residents/patients who need support to lead a more active lifestyle. The idea of the hubs is to provide a safe, friendly and inclusive welcome to those signposted by health, social care or community teams for support to engage in an active lifestyle.

Active Lifestyle Hubs will be geographically spread across Bucks and MK, with the majority in or within easy short travel from areas of health and activity deprivation. We want all residents living within a priority area to be able to access their nearest hub without the need for a car.

Leap will lead a collaboration of partners to identify, establish and connect the hubs across the county and plug them into the health and social care system to ensure key sign posters are aware of the offer and can easily signpost those that can benefit.

Why Active Lifestyle Hubs?

A simplified and established pathway across Buckinghamshire and Milton Keynes will provide an efficient route for residents to be supported into regular movement physical activity and offer a simple, trusted system for health, social care and VCSE professionals to signpost to.
